Ursuline Academy Education Verification Request

The Diocese of Springfield in Illinois Office for Archives and Records Management will provide education verification information for Ursuline Academy alumni upon request of the student.  Education verification information will be released directly to the student or to a third party of their designation.  Education verification consists of the name of the student, their dates of attendance at a school, and their final action (e.g. graduation, withdrawal, or transfer) at that school.

There is a $10 fee for each verification request.

Education verification requests will not be accepted over the phone.

Education verification information must be requested in writing.  Payment must accompany all requests (see methods of payment below).

To request education verification information:

Education verification requests from a third-party verification company must include a signed release form from the student.

Please submit your request using the Ursuline Academy Education Verification Release Form, signed by the student.

A $10.00 non-refundable processing fee and a clear photocopy of an official photo ID (photo ID must include signature) are required with all education verification requests.

We do NOT accept personal checks.

Requests can be submitted in person at the Catholic Pastoral Center, 1615 W. Washington St., Springfield, IL 62702. Cash will be accepted only with in-person requests.
